Consent to Release Forms:

• SSA Release (Doc) - Allows us to contact Social Security Administration and confirm the claimant's Social Security status and Medicare entitlement
• CMS Release (Doc) - Allows us to contact the Centers for Medicare & Medicaid Services should the claim be submitted to CMS for approval
• MSPRC Release (Doc) - Allows us to contact the Medicare Secondary Payer Recovery Contractor to determine and negotiate a conditional payment lien
• HIPAA Release (Doc) - Allows us to contact medical providers should we need to request medical information
